GP Fit Notes
If you’ve been off work sick for 7 days or more, then you may need to show your employer a fit note.
How to get a fit note
Please note that you do not need to ask for a sick/fit note until you have been off work for 7 days. If your employer requests a fit note for less than 7 days, you can direct them to this website.
If you have been ill for more than 7 days, you can request a fit note from your GP by selecting 'Admin Query' on our online AccuRx form.
Once you have completed the form, your GP will get in contact to either issue a fit note to say that you’re fit to work or book an appointment with you. This depends on your illness and whether you’ve been in hospital.
During the appointment, your GP will assess you and give you medical advice. Based on your appointment, they will be able to tell if you are fit for work but might need to make some possible changes, or if you are not fit for work.
School Absence Notes
By no fault of parents themselves, sadly we find that appointments are consumed solely for the purpose of asking for doctors notes. Please can we reassure you that children do not need sick notes for sick days from school. We will not be able to provide them.
If your school insists on proof of a consultation please give them a copy of this letter.
