How to register as a new patient online

Complete the online form to register to become a patient at Pioneer Medical Group.

We may ask you for details of your previous GP surgery and basic health and medical information.

Our catchment area

After a change to the restrictions on practice boundaries, we have been approached by a number of patients asking to move to our surgery who do not live in our practice area.

This is called an out of area registration. for example 

  • Leaving our practice area but staying on our list
  • Live on outskirts of practice area but use our services
  • Commute to Bristol / work near but live out of area.

We do not normally accept patients from out of area as new registrants. If you have already been on our books, we have an established relationship with you, and you then move further afield - assuming your circumstances allow - we will do our best to accommodate an application to remain registered.

You can use your postcode or location to check that you’re within the surgery’s catchment area.

Registering online as a non-English speaker

You can find guidance and advice on how to become a new patient and the NHS healthcare services you can access, if your first language is not English, or if you’re an asylum seeker on the Government website. 

Go to the NHS entitlements migrant health page on the Government website.

Registering as a temporary patient

This can be helpful if you're living away from home but do not want to change your home GP surgery. It will allow you to temporarily use our services and be able to see a healthcare professional at our surgery.

You can register as a temporary patient with Pioneer Medical Group for up to 3 months. 

To register, please visit any of our sites to complete a temporary services form, or call us on 0117 959 1919 for more information.

After 3 months, you'll have to reapply to register as a temporary resident or become a permanent patient.

What to do if you’re registering on behalf of someone else

If you are a carer of young children, elderly adults or adults who aren’t able to register themselves, you can set up an account on their behalf. This will allow you to book an appointment and access parts of their GP online services account.

Change of address

If you are moving house, please let us know as soon as possible. If you are moving out of the surgery’s catchment area, you may be asked to register with a surgery that’s closer to your new home address.

You can complete this form to let us know about a change in your address.

Please also let us know if your telephone number or email address changes.
